​POEM/SHORT STORY ENTRY FORM



Submissions for SCARS - A Healing Anthology is open through December 31, 2022. The book will be published during National Mental Health Awareness Month, May 2023. All submissions must include your name (plus pen name if you use one), address, email address, and signature. There is no charge to enter your poem or short story. Limit three poems/short stories per person. Please include a completed entry form with your submissions. There is no monetary compensation for works published. Authors will be given an opportunity to promote their website, networks, and contact info. along with being able to purchase the book at half the retail price (plus shipping via USPS Media Mail). The SCARS book may be published in a variety of formats including physical, electronic and audio. Proceeds will be donated to mental health organizations such as 988lifeline.org and NAMI.org. You maintain the copyright of your poem and/or short story.
